Sounds like you got a defective bulb. Make sure it is for 120 volts or thereabouts....not 6 or 12.
Yes. It is possible that you jostled the bulb on installation. It the bulb is a screw bulb, be sure it is installed firmly. Before you put it in, have a look at the socket. Is it clean and bright or dirty? If not clean and bright and with the unit unplugged, use a pencil eraser to clean the contacts. Believe it or not, dirty contacts can burn out a bulb!
