This appears to be a Walmart, Amazon, eBay special without a maker website which usually means made in China and sold in bulk. There is limited help here and some videos. Motor and Capacitor wiring all works to the same diagram.

Try Amazon (new things) or e-bay (used things) on your computer for the motor and it's price.
Since these items are likely made in China, the motor may or may not be available.
You can generally find new motors at an electric motor shop, check the yellow pages. However they are many times more money than a new fan. Then add time and your drive to get the motor, my recommendation is purchase a new fan, cheaper in the long run.
Try oiling the pull bearings. If you can determine that the noise is in the motor, try nelsonappliance.com. They have replacement motors for your fan for less than $20.
