In addition to resetting all the camera settings you should also choose the "clear all custom functions" option that is also available from the same menu. Once that's done. Put the camera in Full Auto mode and see if the issue is resolved.

I remember seeing an option to disable the external flash so that the cam will use the in-built flash. Did you try that too?
Oh Dear! THats what I meant! Alright there is one more thing we can try. Remove the battery from the cam. Hold for a minute and then put it back in. Then while powering it on press and hold the on button for ten seconds. That should get the cam to use the inbuilt flash

However, mine is already set to 'Emit' for the internal flash (the other options are 'does not emit' and 'only emits external flash'). I cannot find any other settings for external flashes, so if this is the one you meant, it appears the camera is set correctly for the internal flash to be used. But it is still not working.

Well, removing the battery did not work either.
But I finally got a response from Canon, and apparently there is an issue with the hotshoe safety getting stuck. I will try their tips on getting it un-stuck, and may end up having to take it in for service if none of them work.
