If it prints correctly cold, then starts to fail, check the carrier slide and tube, probably no lubricant there and as it heats up it will bind and fail.
Try running couple of these test, to see if it might be another issue.
It should be printing these test at 1500 Line Per Minute... ?
1500LPM is at 10CPI if you have (Font size: 6, 8, 12, 14) it will make a difference.
Change your driver to Generic/Text, rerun your report... any difference?

I will be glad to answer your question, if you can get me the Specs. for the new postal intelligent mail barcode... the other important question is... what is you current IBM 6400-XXX printer configuration? press the print config, then the Start on your printer. 1st Paragraph is all you will need to give me... Worst case, you might need a differenent Emulation program...
IBM does not manufacture printers any more, they outsourced that to other places like Lexmark & ProPrinter, so we have to figure out which outsource place is relevant & what name they have for whatever corresponds to 6400 driver.

I have come accross this problem on a number of sites. It may be the Paper Out sensor. It works by emitting light from one half of the sensor reflectiong it of the paer and picking it up with the other half of the sensor. When no paper is present, the light cannot reflect and hense a paper out error is reported. I have seen this error occur when paper with dark backing ( like that for bank and tax forms ) is used.
Can you give more detail as to what the problem is?
