As with most games, there are always bugs when Downloadable Content (DLC) is first released. Your issues should be fixed with the next update for Black Ops.

Make sure that the system you are installing them on is activated for your account. If you have issues, Microsoft is actually really kind and easy to talk to about it.
Try going to download it again. XboxLive/ PSN can tell you already bought it so it wont charge you again, but it WILL download everything. Problem solved
Check the inside ring of the disc, is there any cracking? If so the only real solution is to buy another disk but to be sure it's not the system try another black ops disc in your system.
Make sure that you are downloading the actual map pack for the game, and not just the previews of the maps. After you are sure that you have selected the correct item, you will have to purchase it by credit card. Once you have purchased the map pack it should begin downloading immediately. A screen will pop up with the status of your download. Do not exit this screen until the download has been completed. Once the download is complete you should receive a notification and you may then exit the Xbox marketplace and proceed to Call Of Duty where you will be able to play on the new maps.
On PS3 you need to make sure you downloaded the right map pack for the right disc number you have. Take out the black ops disc and look for a code that starts with BLES or similar. Then in the Playstation Store make sure you download the map pack with the same code as your disc.
you need to select the new map pack press x and install it this is done in the ps3 menu, then start call of duty as you would normally .It will now recognise new pack
