I have a problem whereby the top tray keeps coming off of its runners. Either the wheels fixed to the side of the dishwasher fall off or the tray pulls out of the runners. In either event the tray drops down. This is a very frequent occurrence. There seems to be a lot of loose play where the wheels push on to the fixings in the dishwasher side.

There's a very easy solution, which took me a few months to work out! Take the wheels off, lay them with the large side on the table. If you look through the hole, there's a small lip with a gap between it and the wheel. Use a knife or sharp implement to make the gap a bit bigger by pushing the lip slightly to the middle of the hole. Put the wheels back on with the large end towards the wall of the dishwasher. You will hear the wheels click in. Repeat if it ever happens again!

I have the same problem. The little wheels have one flange larger than the other so I try to put the larger side closer to the side of the dishwasher, don't know if that helps or not. I also noticed the plastic is all rough and pitted, I wonder if you can get replacement wheels, whether they would stay on any better? Either way, mine has been doing ok with three runners for a while, I just retrieved two from the bottom of the machine, have no idea where the other three went!
I have solved my problem by taking the screws out which hold the wheels on and putting a small washer underneath the head. The washer needs to be exactly the right size.
