Are you using the Epson TM-T90 driver? When you go to printer settings through Printers and Faxes there is an option to Open Drawer after or at the Beginning of the transaction.
One other way to check if you have the correct commands for it is by checking on printers Manual and Technical documents of it where you can get online. Click HERE. Register and wait for an Email for Username and password.

Attached are the programming barcodes for the LS2208 solve this problem. Print the attached file and scan the barcodes in order 1-10. This will only work on a LS2208. If you scan the Set Defaults barcode it will erase this programming rule.

I have the same problem...Epson does not work with a parallel to usb cable or adapter. If you still want to use this printer you will have to use the parallel port or change the interface on the printer to USB.

Do you power off before you connect all attachment

have you tried different darkness settings? it sounds like you only need to turn the darkness up a little.

your print driver is printing in graphics mode or in reduction mode. disable print spooling, disable A4 reduction or use a faster driver like the epson advanced driver or my fav the generic text only (super fast)
