Sounds like milk steam apparatus is clogged. Try to disassemble this portion and clean thoroughly. Descaling will not help this section.

Try using a finer grind. Remember to adjust grind only when it is grinding. This being the case you may need to do a second brew to test new setting. Mine is set to near finest and I always use fresh beans.

Did you cheak if the spots are blocked did you run some clean water ? The milk further is a different section from the coffee try and consentrat on the coffee part run some clean water is there any noise is the water going in to the trip tray because the spout are blocked ? Well I can't say I understood but two things I can say try descaleing to clean the flowmeter if not I would change the flowmeter

May be blocked or foulty selector or bad commands from cotroll PCB try running descaleing may be spouts are blocked
