The Epson 3490 is a flatbed scanner that has an optional business card/4x6 photo auto feeder. Do you have the optional feeder attached & plugged into the correct connecter on the back of the scanner? You probably need a new printhead, but that problem cann also be caused by the purge or cleaning station not working properly. If you do a cleaning cycle with no carts. and the little pads in the printhead come back with no ink on them then the purge is working and you need a new printhead.

I opened the hood on the machine, and between the first two intake rollers on the bottom, there is a small rubber strip. This strip comes packaged with a sticky yellow tape to protect the rubber. When it was installed, no one removed this tape. After doing so, the machine feeds great. Hope this helps someone else who may be experiencing a similar feed issue.
If you lift the scanner cover, you should see an area on the right end of the platen glass marked at the edge with arrows as Read Area. This is the area used when documents are scanned through the auto feed. Make sure that the glass is completely clean in this area, as any dirt will show up as a line on the copied image.
