I've rebuilt many of these in the past, but never had this problem. it seems there may be a blockage not alowing the primer bulb to function properly. its hard to push in and slow to pop out. it does pull fuel in and out, but it doesn't seem to get through the carb. I've taken it apart cleaned it three times. there seems to be no visible problems with the carb body. could there be an issue with the metering plate between the primer and carb body?
Try to pressure test the carb to see if you have an internal leak. If it won't hold pressure then it is junk and needs to be replaced. If it holds then double check that you didn't accidently put the gaskets and diaphragm in wrong. If it holds and still doesn't work right replace the carb anyway. They are relatively inexpensive

It could be an air/petrol mix that is wrong that would explain why it works with exhaust off

You definitely have carb. problems. The fuel probably varnisized. In this case, you may have to place the carburetor in carb. dip for a few hours.
