000000 is the default code for the master reset and clear.

It sounds like the phone itself is bad. You need to take it to Verizon and swap it out for a replacement. Depending on how many times you've had the problem with the same phone you may be lucky enough to have them upgrade you to a different phone model. There is no upgrade that can be done for this phone to fix it.

You can try to do a software upgrade to it in the Verizon store if Verizon is your provider, they will also have a NEW battery for you to try and if it is under a year old they can swap it right over the counter under warranty for free.
