1. if you've already registered the sim with the network- go to home page on web login to your account click on the help section click on unblock my sim, enter your mobile number & hey presto PUK code appears
2. ring the network & ask them for your PUK code

Your nokia 6020 doesn't have problem. It's your sim card. If you still have your simcard packaging, the PUK code is written their inside it's brochure. Or you can also go to your mobile phone network and ask its PUK code. Or simply buy a new simcard with the same network company. Thank you

Just call your provider and tell them you need a PUK code, they will be more than happy to give it to you.

If you log onto your companys website under your account and go to the option that says devices you can get the puk code- If you enter in too many numbers it will permanently block your sim card and you will have to buy a new sim card from your cell phone company- they are about $20

No the Default PUK and PIN codes are not set by you, these codes are Service Provider Dependent (SIM)
All you have to so is to call your Service Providers Customer Support (Like of T-Mobile, AT&T...).
And ask them about the PUK and PIN code, they will first Authenticate few things about the Ownership of the SIM card, and will give you the codes, in return.
