If there is no water on the floor, it would have to be that the water entering the washer is going through the drain tube because it is too low, or for some odd reason the drain pump could actually be running, pumping out the water as its entering the washer. If there is water on the floor, there could be a leak anywhere along the drain tubing, the drain pump itself, or even the drum/basket seal.

Ensure that the waste pipe of the washing machine is higher at one point than the drum of the machine, this will prevent water running back into the machine. Cold water won't stop before rinse cycle

if it fails to stop running only at the rinse part of the cycle it is probably in the timer as the solenoid valve has to cut the water off when the washer fills for the wash cycle too. fills for wash cycle and cuts off water, washes and drains, refills for rinse cycle and water just keeps running? does this every time you run it? that's in the timer. bad solenoid (water inlet valve) would keep running even on wash fill up. hope this helps.

Toilet fills up and water keeps on running (toto 6.0 lpt)

If any toilet runs (continues to fill) when it is not being used, there are basically 3 reasons. the most obvious is that water is leaking out all over the floor so the fill valve thinks the tank has not reached its volume of water. Next, the rubber device that keeps the water in the tank when not being used (most commonly called a flapper) has warped from time or chemicals in the water (NOTE:chlorine eats rubber for breakfast daily), and and allows water to seep past it, which lowers the water level and triggers the fill valve to come back on to top things off.
The third reason is because there is particulat in the fill valve or mineral buildup and internally it doesnt always properly seal so it can run intermittently or all the time.
All the parts on the TOTO ultramax are warranteed for 5 years so keep that in mind if it applies.
Remove the tank lid when you hear the fill vave working and look at the water level. If the water level is below the top of the white plastic tube that the flapper is connected to (often called the overflow tube), then you have a bad flapper. If the water level is going over the top of the tube then you have a defective fill valve.
Out of warranty fill valves can easily be replaced with a Lavelle "Quiet fill" available at most hardware or plumbing stores or online.
This valve is what Toto uses in most of their toilets and it is only the removeable cap that is different (subliminal advertising maybe)

The water filling the tank does not turn off, it keeps running

It sounds like the fill valve is not operating correctly. Remove the fill valve which it the tall item inside the tank where the water runs through and replace it with a universal replacement part from your local hard ware store. Take the valve to the store so that you can get the proper replacement piece.

Washer won't fill

On the drain valve there must be some dirt that does not let the valve close or the valve must be defective and needs to be replaced. By the way you did not mention the make of the washer.

Washer constantly fills with hot water even when turned off.

Sounds like a bad mixing valve. To verify that it is the valve you need to turn off the washer when it is filling. If water keeps running, replace the valve, Cost is under $50. for most washers.
