You need to have the transmission control module (TCM for short) tested for fault codes, this transmission is shifted by solenoids and they set fault codes when they malfunction in most cases. Other causes are sticking valve body check balls or internal pressure leaks in the hydraulics. Have a transmission repair shop drive it and see if they agree with me on this preliminary diagnosis. Hello there, I have a 91 dodge grand caravan and by the looks of it, you used the key on, key off, key on, key off, key on method to get those codes. If so, 12 is one code(number of key ons since last fault)(direct battery input to controller disconnected within last 50 too 100 ignition key ons), And for code 55, its just the completion of faultcode display on the check engine light. i recommend going to walmart or car toys and buy a wiring harness made for your vehical that way you dont have to match with the stereo wiring sometimes accurate diagrams are hard to find.

It is the Upstream Sensor. 100 Codes ending in 30 thru 35 are dealing with the Upstream Sensor. 100 Codes ending in 36 thru 41 are dealing with the Downstream Sensor.
