My '99 Ford Windstar has been having numerous issues. At first, during cold weather, I have to preheat the vehicle for about 10 minutes before pulling out of the driveway. Secondly, somewhere between 45 and 50 the van's front end shakes a bit when I press on the accelerator, and again somewhere between 60 and 65. Most recently, the van won't push past 45 MPH anymore, and I can barely get to work. Any ideas?
The engine computer is in the limp in mode. Have it tested. Sounds like you slung a wheel weight. Have your wheels balanced again.
1st have the engine control module scanned for fault codes and run a test called an engine running self test, next check the fuel pressure, it should be about 46 pounds under a load. If it low try replacing the fuel filter (located under the car on drivers side)

The desciption you gave is exactly how it feels when you have an axle with a bad inner CV joint. It won't make a "clicking" noise like when you have a bad outer CV joint, but will shake pretty bad on accel. If you crawl under the car while it's on the ground (don't lift it) and grab on to the axle close to the inner CV joint and wiggle it, there shouldn't be very much play in it. If there is play in it, replace that axle.

it could be a motor mount check the condition of it or it can be that something with the suspension might need attention . . those things can cause vibration at certain speeds

Misalignment of the belt, incorrect belt, worn or loose tensioner, Too tight of an adjustment (many service centers do this on purpose so when the belt breaks in it becomes properly adjusted, I do not believe in this philosophy), The belt is actually one long belt called a serpentine belt.....
