I have tried all of what you suggest below and still receive the same results. What would this have to do with the heads? The printer has no problem printing a test sheet.
As soon as the paper loads, the file I'm trying to print disappears from the printer's documents.

The spooled files are not getting to the printer, do the following in sequence:
Start checking usb cable, test it by using a different one.
Check that no device close to usb can create an electromagnetic disturbance, a power adapter can do this.
Disconnect printer from power, Computer from power (unplug cables form power outlet) and printer from computer for 10 minutes (this will reset printer and get rid of electrostatic charges on PC usb).
Connect everything back and try.
If none of the above worked disconnect printer from usb,
remove printer from 'installed printers' in Windows (or whatever OS you are using)
Reinstall driver from printer disk (printer must remain unplugged).
Plug back printer only when required from printer software or after driver setup.

The MD-1000 is not a USB printer.

Have you downloaded the Alps drivers for the printer? The microsoft driver included in some versions of windows should NEVER be used, it's junk.
Go to http://www.groups.yahoo.com, and search for the Alps group. The drivers are in the files section.
Skymail.....this is a known problem with the Microsoft drivers supplied with windows.
If you want to solve the problem, download the Alps drivers from the aforementioned Yahoo mailing list. Since Alps USA ceased support for these printers, and removed the drivers from their website, this is one of the only places to get them. Otherwise keep trying with the M$ drivers, but you will not get ANY joy.

And Skymail....I forgot to add - the Alps drivers will add all sorts of functionality that the microsoft drivers lack, for example Overlay, etc., etc., etc.
Follow the procedure in the technet link above, and you will be printing again in no time, WITHOUT having to re-install windoze.

What usualliy happens when is like this is that printer cannot understand the files because either the driver on Windows is not communicating properly with the driver or the file arrive corrupted on begin of file, then printer starts printing blank pages.
Sounds it is the driver or system. If you alredy installed driver properly it could be a Windows fault. Try installing it on a different computer , (install driver BEFORE connecting printer). IF it works, then is a problem with Windows on other computer , in this case you will need to reinstall or fix windows.
In some rare case can be defective printer usb slot. You verify all this connecting to other PC
No matters if drivers were working before. Like any other software a device driver can start malfunctioning simply because a windows setting has changed. A device drivers is interfaced with the operative system in a complex way, if windows has a problem a driver may stop working or require an update driver, for example to work along the latest windows update.
I can only suggest here the general procedure to troubleshoot this kind of problem, that in any case is what I would start doing if I had my hands on your printer.
Install the printer on a different computer, to ensure that is not a printer fault, does not matter if it prints text page, a fault for example in printer bus will cause the printer permanently staying offline.
If the printer works on a different computer is the driver that is not working along with windows.
Try first removing installed printer and reinstalling the driver.
If this does not work, easiest thing to do is reinstalling windows.

It was working fine before with no other drivers than what came with it. There's no reason it should need any new drivers.
