To get the PUK code please call your service provider. They wil ask for your informations to check weather the phone belongs to you,please provide correct information. they wil provide the PU code.

if Puk is blocked, then there is no way round. however, I should inform you that its your sim card not your LG that stores puk. so you have to contact your sim card provider for a sim replacement.

The puk code is sim card spesific it comes with the sim card when you get it or you can get it by calling your phone service provider of the sim card to get the puk code.
if you have entered the puk code wrong too many times you will make the sim card useless and you will have to contact your phone service provider for a new sim card, it will probably cost.
