There are several reasons that can cause mattress sagging including improper, improper center support, a damaged or bent frame or a broken box spring.

Trying to avoid the above will help you reduce you mattress sagging and maintaining your mattress comfortable.

When I buy a new mattress do I also need to buy a new bed frame?

Technically no. You can buy just a new mattress, but some of the damage done to mattresses ie the indentations are caused by insufficient frame support. So check your frame to make sure it's in good condition with nothing broken.

My sofa bed is sagging in the middle and it's really not that comfy to sit on. Is there anything I can do about it?

Don't rush out to buy a new sofa bed. You can reinforce the one you have with plywood. You will need to measure under the mattress and have pieces cut that fit in horizontally but it's not a hard job and should make a big difference.
