Start by checking your fuel filter. Next check to see if measured pressure at fuel rail is to factory specifications. Your fuel pump could be starting to fail causing low pressure under load. Your fuel injectors may need replacing or at least have them cleaned or replaced. These are the first places that I would start given the symptoms you describe.
MAF mass air flow sensor.

clean out ur throttle body, just follow the hose from the air filter to the throttle body. Need 2 people to do this. Don't remove the hose completely from the throttle body. Just enuff to be able to spray ur intake cleaner into the throttle body. Do this especially if its worse when the motor is hot, as this would confirm the problem.
SOURCE: 1998 GMC Jimmy, bogged down, stalled and won't sart back up
does it have spark? you did verify fuel pump working properly? could it have a clogged converter? is the engine lite on?
SOURCE: hesitation, stalling like out of gas
Clogged Catalytic Converter. What happens is as you push down harder on the accelerator you force more gas into the system causing a slightly richer mixture and more exhaust flow is neccessary. The easiest way to look for a clogged cat is when it's dark out take it for a ride causing it perform it's problem look under the vehicle if the cat is literally glowing red hot you either have a clogged cat or an extremely rich fuel mixture.
SOURCE: engine dies after idle
have you ever had the injectors checked. if not try runing some fuel injection cleaners through it and see if that works

check idle air control, tps & map sensor. As usual with electronics, test test test before buying anything/
You can in most cases repair this slow speed stall or unstable idle by cleaning the engine throttle valve and the idle speed control valve using a product called CRC Spray Throttle valve cleaner. Spray the cleaner into the air intake of the engine with the hose removed from the throttle body, the engine will try to stall so hold up the idle speed manually. Once you have used up about 1/2 can of the cleaner stall the engine out while spraying and turn the key off, disconnect the negative battery cable for 5 minutes and then reconnect and drive the car for a about 25 miles of varied driving modes. If this doesn't cure the problem you may need the fuel injectors professionally cleaned by the dealer service dept.
