If you cant login to your SIM card and it has a password:
If your SIM card is blocked from to many tymes of typing the wrong password you will have to go to your phone operator and tell them that you blocked your SIM card
If your SIM card isnt blocked :
Step 1 . Turn off your phone
Step 2 . Get out your SIM card out the phone
Step 3 . Blow air in to the place that your SIM card should be (you can use your mouth or can of compressed air)
Step 4 . Blow your SIM card with air (you can use your mouth or can of compressed air)
Step 5 . Insert the SIM card back in to place
!!! If this fails you will have to visit a cetificed phone service !!! If you made your SIM card in to Mini format theres a chance you damaged your SIM card
PUK code gets blocked because of multiple incorrect tries to unlock the sim card's password.
Now what you would have to do , if you are getting a dialog box like PUK2 code required , is that you can call up the sim card's provider and ask them to give you the PUK2 code and check if it works , as it should work normally.
Now if your sim card has been blocked it should say SIM card blocked and Not PUK code blocked, then all you need to do is get a replacement of your sim card as it cannot be unblocked once it has been blocked.
This facility is provided by the Sim card provider for security reasons , in case the SIM card gets lost and falls into someone's hand.

To unblock your SIM card:

Enter a new four to eight PIN > OK
Re-enter the new PIN to confirm > OK
To edit your PIN:
> Settings > the General tab > Security > Locks > SIM Protection > Change PIN
Enter your PIN > OK
Enter a new four to eight digit PIN to confirm > OK
To turn the SIM card On /Off
> Settings > the General tab > Security > Locks > SIM Protection > Protection and select On or Off
