You need to change your brake pads/rotors on the front of the vehicle. Somehow, it sounds like your rotors have become scored and you need to change them out to get the shaking to go away. You also need to change the pads since the pads on there now are worked into the old rotors. Change those out and your brakes will be good to go. run the fault codes to look for injector problems
check injectors for bad tips
check fuel pressure to injectors
check for vacuum leaks with the foot on the brake ( booster leaks --check by applying the hand brake with the foot off the brake and if the problem is not there it is the brake booster faulty)

If the steering wheel shakes while applying the brakes, you have warped brake rotors, and they need to be replaced. A shake all of the time may also be another issue, as in tires out of round, out of balance, or bad tie rod ends.

Probably a disc brake pad scraping on a warped rotor as it goes around. When you apply the brakes the warped rotor causes the car to pulse or shake as the rotor goes around and applies the brake unevenly. You need a front brake job.
