is this the blower motor fan for you heating and A/C or the cooling fan cooling fans have a relay blower fans are fused but controlled from switches inside as well as fuses
It seems you are low in coolant. The temperature sensor has air to sense instead of liquid and temperature gauge will be low. Try to fill up with coolant but sometimes it's hard to get rid of air from the system. Have you changed the engine thermostat?

I'm assuming the heat for the inside of your car. Does it work at all? On any of the speed setting? If it doesn't check fuses, check for power and ground at fan. Or if you can see it just bang on it with your fist sometimes that will get it working temp.
Does the fan work on A/C? If the fan isn't blowing, check the fuse. Heat when going down the road is simply an indication that the air from outside is coming through the system. If fuse is fine, blower motor is probably out.
There is a resistor block in the heat duct that controls fan speed usually when these go out the fan will quit on one speed but work on others until finally all speeds quit,also there is a relay that powers the fan it is under the hood in the relay box
The contacts on your fan switch are bad, or you striped the knob and it won't turn the switch on, or the big bad one...your fan motor has decided it needs a vacation.
