The mixer is stereo and the amp is mono. The line level out of the mixer MAY overwhelm the KB2 regular inputs so try running the mixer into the PA input jack.

Take the main outputs into a stereo input (or a pair of mono inputs) on the Peavey - without knowing which Peavey I cannot tell you further info re set-up but as you'll be sending line level signal you'll need to back off the gain on the channels you input to on the Peavey

Two things... Use BALANCED LINES, either XLR or TRS... AND power the mixer from the same receptacle or power conditioner as the speakers and the sub.

How can I set up my mackie 1402 mixer so I can have (2) monitor mixes

This unit has 2 AUX sends, both of which can be used as monitor mixes. The top controls on each channel determine the pre-fade level for each mix.

Conenct a cable from the AUX out (either 1 or 2) to an amp. The only restriction is that the level control is a knob rather than a slider.

If you havent ground looped the chanels. Most of the time the chanels need to be cleaned.

Yes, the ribbon cable is the 1st thing I would check. You can check the ribbon like this: Disconnect said cable from the channels. Connect cable connector from a known good channel to the problem channel. If channel sounds the same trace a signal through the channel op-amps.

No volume control with 2 faders on Mackie CR1604 mixer

If all other functions work for those channels, the most likely problem is the fader itself. These can fail and operate more like switches rather than faders. Without witnessing the behavior, I can not state that for sure. While these mixers are not the hardest to work on, they are time consuming. The faders can be purchased from Mackie directly. If you need to have this repaired, any shop that repairs audio gear should be able to erpair this for you. Where are you located?
