If you wanted to re-activate then is it so that your sim is disabled, if so you need to contact for the same to the sim service provider. For the phone you need to go to settings and reset ( option is RESET)

there is absolutly noway of getting the puk code without contacting your phone service provider not the store.
you have entered the pin code wrong 3 times now you are required to enter a puk code to reset the pin code.
The puk code is sim card spesific it comes with the sim card when you get it or you can get it by calling your phone service provider of the sim card to get the puk code.
if you have entered the puk code wrong too many times you will make the sim card useless and you will have to contact your phone service provider for a new sim card, it will probably cost.
