The phone asks for PUK code ( Personal Unblocking Key) if a wrong PIN code is entered repeatedly
PUK code is related to the SIM card you are using with your phone and not the phone itself.
The code can be supplied only by the cellular service provider. So, kindly call the customer support of your cellular service provider using another phone and they will provide you with the code free of charge after verifying the details.
Please note: Entering a wrong PUK code ten times in a row permanently disables the SIM card. So please be careful while entering the PUK code.

puk code comes with the service provider. It is different for each sim card. All you have to do is to ask the service provider for the puk code and they will provide it.
Pin code also comes with a phone number. That too is given at the time you buy the phone. And if it is incorrectly entered the phone asks for a puk code. So the pin code can also be obtained from asking the service provider.
