Turn the thumb knob on top of ratchet to opposite direction and then try it in your hand first, now I have seen cases where if you are in tight quarters that the ratchet will change directions by itself because the knob rubs on something and causes it to reverse. good luck
On the end of the ratchet head there is a little lever that can be moved to an alternate position that allows the ratchet to tighten/loosen in the opposite direction. You should be able to flip it with your thumb.
The rathet will continue to "ratchet" as it turns, but it will be working in the opposite direction, i.e. clockwise instead of counterclockwise, so you have to change the lever position each time you use the tool to either tighten or loosen the spark plug or nut. It clicks so you can return your hand to a good position to continue tightening, without having to go around in circles.

I apologize if my answer was not to the point.
Perhaps the difficulty is that there is not enough friction in the spark plug threads to allow the ratchet to turn. In that case, you may need to use an extension so you can hold the extension and keep the plug from turning in the other direction to get the ratchet to ratchet.
