The internal (replaceable) power supply has failed. If you are good at fixing things - soldering, etc. - you can replace the power supply with a Digikey part #285-1779-ND (KMD15-55)
Just be careful and mount it to the top aluminum cover with epoxy.

I ordered the Digikey part. The high voltage side is self explanatory, but the low voltage side on the Extron has three wires. Black, red, orange. Which goes to ground, +5 and -5?
Do a Digi-Key search of part # KMD15-55. The KM series power supply instr manual .pdf is available from that page on Digi-Key. Page 13 gives the pin-out of the new unit. Write down the connections from the old power supply to the Extron circuit board. I mounted the old power supply with epoxy and these have held very well. I have repaired 6 of these Extron RGB 109xi units with these TDK-Lamda power supplies and had no problems. Richard C.
