This is SIM card related / specific. If you take the SIM
card out of the phone, the phone will work with
another sim card.
The puk
comes up when the pin code has been entered wrong 3 times. To get the puk
code for your sim card, you have to call the service provider of that
sim card. They are the only ones who can provide you with the right puk
code for that sim card.
The puk
code may also have came with the documentation of the sim card, if its a
prepaid card the Puk code
comes on the documents or on the box of the product.
if your unable to obtain the puk
code, get a new SIM card. Also entering the puk
code wrong too many times will eventually ruin the sim card forcing you
to buy or contact your service provider for a new sim card.

If it's your pin number you forgot, phone your service provider and get your PUK code. Write it down and check with them that you have it right. There are 2 PUK codes, one for PIN 1 and one for PIN 2.
When you have the PUK code, enter any wrong pin 3 times and the phone will lock. When the phone is locked turn it off and then on again. It will tell you that the sim card is locked and ask for the PUK code. Enter the PUK and it will then ask for your new PIN. Enter a new code and it is now changed.

i think you blocked the pin code by entering the wrong pin code for 3 times so it is asking for the puk code ..
to get puk code you must follow these steps :
1. call to your network provider customer care
2. tell them that your pin code got blocked you want puk code
3. if they ask any details about the sim card owner you answer that one ...
4. then make a note of the puk code and enter in to your cell
note : if you enter the wrong puk code for more than 3 or 10 times your sim will get fully blocked you must purchase the duplicate sim card or you must contact the network provider shoppe ...
Every PUK code is unique so nobody can tell you your PUK number, but i will tell you what you can do! 1)
If you still have your packet that your Sim Card came in, you should
find your PUK number in there, if you don't have the packet, please
read bellow.
2) You must get a phone that is on the same network
as your phone and call the networks Customer Care and ask them for your
PUK number, they will ask you for your phone number, then they will
tell you your PUK number. Once you have done this, you will be able to
reset your Pin number so you will be able to go into your phone again.
