fixya site is not a pharmacy
Next---- this site is not a secure site and as you have made the serious mistake of name, DOB and SS number , you have set yourself up for serious identity theft
In future just ring the pharmacy yourself
Not sure exactly what your question is. I think you are asking what "case sensitive" means. If so, it means that whatever password you choose (except for your name or dob), if you use uppercase letters in it, then you must use those same uppercase letters when you log in.
For example, if you create a password like this: paSswoRd, then when you go to log onto your site, you will need to fill in your username, and then your password---"paSswoRd", and not "password" or PASSWORD.
Hope this answers your question.

Most codes on phones have a generic code of "0000" or "1234" or "123456". try that as the PUK code. If not, contact the manufacture and they can give you the unlock code.
