As far as I know, the puk code is the 2nd level security code of your SIM card, not your phone's. It is invoked if something goes wrong with your PIM code. It should have come in the plastic card, the one you detach the SIM card from. If you haven't kept that card, you should contact your network carrier, to try to get that code back.

PUK code will come with network provider. So you can call to network provider customer care and they will give you PUK code and PUK2 code those will helps you to unlock your phone.....Hope this will helps you.....
The PUK code is a result of entering the wrong sim pin to many times. Just call your provider or log on their website and they will give you a PUK Code. It's a security measure, and a pain in the neck. I know AT&T has a PUK request on their website.
There is a default sim pin code ask them for that also when you talk them
Call your provider and tell them you need a PUK code for your phone. they will ask you for information on the phone and give you the code all it is, is the sim card lock. you entered the wrong code to many times and it locked.
