The PUK code can be provided only by your service provider, you have to provide your IMEI number by calling them, they will ask a few security questions and they will provide the PUK code.

Hi,there are two ways to get the unlock code for your phone,you either call your service provider who sold the phone to you and request for the unlock code,which will be given to you for free,or you visit this website;www.unlockphone-free.com,or www.codeunlock.com.where you might be asked to pay a token to get it unlocked.
