It Idles fine when I adjust it but when I start it up with the choke and let it sit till it sounds like its ready to ride. I put the chock inn and the bike starts to ride horrible. I cant get the bike to shift into a different gear without it sounding like its going to blow up. I have been using the little screw to take the idle down but hasn't worked.

Check the gears mabey they are skipping when the bike goes to fast. like at 3/4 throttle theres to much torque an it just wont catch.
SOURCE: idles revs by itself
You have an air leak in the intake system. The fuel mix is getting lean from excess air mixing with the gas. New gaskets and seals between carb and head will usually take care of the problem. Any oil stains around the base of the cylinder head? Head gasket going bad.if there is. The gasket could be the leak. Time for a valve job? Air getting sucked in past an exhaust valve or two? Have the compression checked. Find the leak(s) and you find the fix.

SOURCE: I have a 1997 tw200. It runs and idles fine, but
Sound like it could be a problem with the side-stand cut-off switch. (It is to prevent you riding away with the side-stand down.) They are notorious for getting crud in them and sticking.
Hope this helps!
SOURCE: yamaha tw200 2003 dies giving gas. idles fine. wtf!
You are getting enough fuel. ck for trash in your carbs

gear selector may be jammed inside the casing or broken
check that the shifter is tight on the shaft!
check clutch adjustment
try rocking bike back n forth without using clutch while you try ******* the gear lever it may come unstuck
but get it looked at if it frees up!
