Just once try giving 00000000 (eight zeros) as PUK. if it works, thank me, if it does not then please call up the helpline of your service provider and ask for a new SIM for a small charge to retain the phone number. What happens is that wrong PUK locks up the SIM and it has to be replaced. PUK and PIN are two security features provided by the service provider. The defaults are 0000000 and 12345 respectively.
