Your phone will ask for PUK code when you entered the PIN code wrongly several times depending upon the mobile make. The mobile asks for PUK code so that unauthorized persons do not use your mobile for wrong cause.

sir please call to the related simcard customercare and say the last 6-digit number which is present before the simcard.then they will give the pukcode number to unlock the cellphone........

u need to contact ur service provider, call the customer care, they will ask u for ur sim card number which is printed on the top of sim card and will ask u for some personal information regarding ur number. after getting all information they will provide u ur puk number.and make it sure don't put puk code wrong , if put 10 time wrong then u will loose ur sim card and number u will need to issue new sim card with same number but u will loose ur contacts and messages saved in ur sim card.best of luck
