PUK means "personal unlocking key" .
The PUK code is asked if you enter a wrong pin 3 times.
The PUK code is a code associated with the SIM card and not with the phone.
You have only two ways to get rid of the PUK code. Either you replace the SIM card , or you call phone provider that issued the sim cadr (eg. At&T, Virgin, Tmobile , Vodafone, Telecom etc.)

To get PUK code please contact your service provider
through phone or go directly to their office. They will provide you the PUK
