Egr valve stuck closed. needs to be open when the engine warms up to keep the engine running
One of the egr components is at fault so have it checked out for there are 4 componets to it

Won t stay running when first started. runs fine once its warmed up

This sounds like the temp sending unit for the ecm is bad. There are 2 temp sensors, 1 for the light/gauge and 1 for the ecm. The sensor sends a signal to the ecm to tell it engine temp, the ecm them adjusts fuel mixture for cold or warm engine.This works much the same way as the choke does on a carb for cold starts

First i would check the crank sensor by looking for rpm's on the tachometer or scan tool if you see any rpm's then crank sensor is good.Then check for leaking injectors and if they are not leaking then you will need to check the ECM it may have failed.

Then you have a start up problem and it reside with the cold start valve,The cold start valve operates even if the vehicle is warmed up,the cold start valve can be plugged up,or even have a bad connection,The cold start valve operates by a thermotime switch.This switch located on the near bottom of the radiator may have gone bad or disconnected,Here is the easy cost effective route, replace them both,it will cost more to trouble shoot it than just replace it.

Sounds like you have the famous cam position failure.Dont rely on a code it wont give you any By the way does the idle roll and surge ? Then its definately your cam postion sensor

Runs fine when cold, not when warm though

The next time it won't start check the fuel pressure you should have 48-55 psi with key on and engine off.
