Will not recognize card reader
Make sure Windows Update service is running, then unplug and replug in your USB cable. If it doesn't automatically install the driver for it, select the option to search windows update for the driver. This should fix the problem, or you may search windows update alternative updates for a USB update.
There are no drivers for this product outside of the windows USB drivers. Any website claiming there is a driver for it is more than likely offering a you a virus.

Try NOT connecting your camera to your computer.
The best way to download pictures from your camera to your computer involves removing the memory card from the camera and plugging it into a card reader (either built-in to the computer or connected via USB). This is likely to be faster than connecting the camera to the computer, and won't run down your camera's batteries.
Once the card is plugged in, it will appear to your computer as a removable drive. You can use the operating system's drag&drop facility to copy pictures from the card to the computer's hard drive, the same way you copy any other files. Or you can use any photo management program such as Picasa.

How to put photos onto flash drive?
If you are using a Windows Computer, when you plug in the flash drive into USB port, you should get a message on the screen that says that you have installed a new removable drive. It will ask you what you want do next and one of the options would be to copy your photos. Note which drive letter the new drive is using - if you want to copy files manually, open My Documents, select My Computer on the left side panel, and select the drive that matches your Fllash Drive. When it opens, you can open. A second window to the location where your photos are, and then just copy and paste, or drag and drop between the two windows.

You don't. The Zeikos attaches to the front of lens with a 52mm filter thread, which then attaches to the D90. If you have the 18-55mm lens, for example, this will fit onto the front of that lens, effectively converting it to a 36-110mm lens. If you have the 18-105mm lens, this won't fit onto it.This device is really intended for use with cameras that don't have interchangeable lenses. With your D90, you're much better off using another lens instead of putting this onto the front of another lens.

No. It's an optical fault and is either a manufacturing defect or a design flaw.
Such problems are VERY common with ALL no-name brand supplementary screw-on lenses and not entirely uncommon even on branded similar models costing vastly more and intended for a specific make and model of camera.
Supplementary lenses are simply a get-you-by compromise, they will never be anywhere near as good in terms of image quality as a lens designed to be a wide angler or telephoto from the outset but will allow you to capture image which may otherwise be unattainable without a lot of additional expense and equipment.

It's a cheap(ish) generic screw on lens multiplier and so compatibility with anything it's screwed onto is a matter of pot-luck, especially as the conversion factor is so high.
There is nothing you can do, other than to try alternative brands (preferably not exceeding 1.4x) or to obtain the expensive Panasonic version which is designed to be compatible with your camera. Even the official product will produce some distortion (usually barrel distortion) and most likely a fair bit of chromatic aberration. It's just usually less adverse than with the generic products.
