Most likely the installer did a horrible job of installation, poor connection are common, many time the wires are just twisted together when they should be soldered and shrink tube insulated. I would check all wiring that was installed.

Could you please be more specific? I'm not sure if you're looking for a wiring diagram (if so, I have one posted below), or if you're having a problem with grounding or other wiring issues.
