PUK code is a security code for the card and not phone, so that information about your PUK code can only get from the network operator whose services you use. Otherwise this code is used for unblocking the PIN code, and occurs when it is entered 3 times wrong PIN code. If the PUK code is entered wrong 9 times your sim card will crash. Calling the phone company is the standard procedure to unlock a SIM card, however, if you do not want to call the company you can just replace the SIM card with one compatible with the phone, as PIN and PUK codes are related to SIM, and not to the phone.

If you have lost your pin number, the only way to rest the PIN is either using a different SIM card, or entering wrong PIN three times, triggering in PUK code, and then calling SIM card phone company to unlock the PUK.

A lost PIN cannot be retrieved, only the PUK can be discoverd, but you will have to call the phone company.
