It would appear that you have entered the sim pin code incorrectly 3 times so it has locked.
To unblock your sim card there are 2 ways, Enter the PUK code. The PUK code would be located on the plastic card in which you took your sim card off, If you no longer have this however, Best option is to call your service provider and they are able to provide you with your PUK code.
PUK stands for Personal Unblocking Key. This is a unique eight-digit code specific to your SIM card. This is required when the PIN number to unlock your mobile phone is entered incorrectly 3 times. You are then given 10 attempts to enter your PUK code. If an incorrect PUK code is entered after the 10th attempt, you will block your SIM card permanently and will require a new SIM card from your mobile phone provider. PUK codes can be obtained through the following ways - The PUK code is in your SIM card documentation Starter Pack. If you do not have this then - Please contact your phone service provider. Your cell phone service provider can give you the PUK code once you give them your account details and they have confirmed your identity. Hi, if you enter wrong PIN for three times, you are then given 10 attempts to enter the PUK. (it is printed on the voucher of sim card when you have purchased the sim)
Your sim card will be blocked permanently if you still entered incorrect PUK code after the 10th attempt.
And if ever you lost the voucher you have only 2 ways to get rid of the PUK code:
1. Replace the sim card or
2. Call the phone company that released the sim card. They will give you the PUK code once you give them your account details and they have confirmed your identity.

at&t has 2 puk codes per sim card but if that is not working u will just need to buy a new sim card and a at*t store can transfer your sim imformation from your old sim to the new one and new sim cards are $25+ tax
If you log onto your companys website under your account and go to the option that says devices you can get the puk code- different than a pin code. If you enter in too many numbers it will permanently block your sim card and you will have to buy a new sim card from your cell phone company- they are about $20......... Personal UnlocK code is a code set by the phone service provider to prevent unauthorised usage of phone's pin number.. There's only one way to get it.. Call your service provider's helpline and they'll help you for sure..
