The disc is either scratched too much
or
the xbox dvd lense is dirty
if other discs work then it is that specific disk
try cleaning the disc with a micro fibre cloth
start on the inside and rub outwards towards the edge of the disc
you can also buy a lense cleaning kit, you put a solution on a disc and put it in the drive
it should help to clean the laser

The first thing to do is to check the game. Are there any surface scratches? Are there deep scratches?

Make sure your box is not blowing really hot air and nothing is right behind it blocking it. I get disc read errors when its hot. Turn it off for a few minutes and let it cool down. Consider the xbox360 fan add on. From what your saying as long as it is not consistant it is just being TEMPramental . ALWAYS turn it off when your not using and be real nice to it. If the problem is consistant go back to solution 1 as a new or fixed xbox is better than one that may be running into problems.

theres a known problem with the hitashi dvd drive in the xbox 360.
the only solution to this is a home repair job, look up on youtube on how to do this, although this isnt recommended as it voids warranty.
so the best way is definitely just to ring microsoft and send ur console in for repair.
