Use your recovery or operating system disc to run a repair, type in chkdsk /f /r. This should take care of it, however, this could also be a failed hard drive. If the repair doesn't do it. Then reformat the drive and install a new operating system. If you have any issues during or after the format or install, then the hard drive has failed and will need replaced. When formatting, do a full NTFS format, if the drive will not format, then it's done.
Boot using Recovery CD and run chkdsk /R C: to fix errors then reboot.
You may have to install a 2nd copy of XP which will trash the first copy but at least leave your data files intact.
