Your memory card needs to be replaced. Using the AR Max Evo to format your Memory card can sometimes leave bits of information that cannot be removed, and also deletes the firmware of the card that allows the PS2 to read it.
There is no way for you to fix it, unless you exploiting the card with Free_MCBoot. It'll reformat the card if it detects it, and allow you to install the exploit on the card.
For legal reasons, I cannot tell you where to obtain the files.
I have read that some manufacture PS1 memory cards will not work properly on the PS2. I would suggest trying another PS2 to see if it's a problem with your specific PS2. If the memory card doesn't pop up on the browser screen of another PS2, it could be defective or incompatible. If it does work on another PS2, it could be something weird going on with your memory card slot on your PS2.
that happenes to me all the time, wat i usually do is pull the card out, blow into the ports, then put the card in. wen the card is in, wiggle it from side 2 side and your ps2 should pick it up
Are you inserting a PS2 or a PS1 memory card? PS1 games can't use a PS2 card because they don't "understand" how to read them. To save a PS1 game on a PS2 you will need a PS1 memory card.
I hope this helps, I assume this is what you are referring to with your question.
