Go into the phone settings and in there under Phone INfo, it will show you the phone number it has assigned, try using the last four digits, also try 1234, As a last resort, you can reset the phone to factory defaults.
Also, you could download BITPIM ( its free, google it) and try hooking the phone up to your P.C. and using the program to reset it. You will need to buy the verizon sync package to get the cable and drivers, you need to install the program in order for it to install the drivers that you need to use with bitpim.
