insert the card into the pci slot on you motherboard turn your computer on the found new hardware should activate and ask you to install the drivers/software for it

That makes sense, but I've had the same setup for the last 4 years and it's just now giving me these issues. I was leaning towards the motherboard going out, but I figured I better ask before replacing random components. Thanks for the assist though!
