If you still have the box it came in, it would be listed on the side. You can also contact your provider and see if they keep that on record. You can also report it to them as being lost, however that doesn't prevent someone else from using it. Other than that, there maybe no way of getting it.

Contact your network provider. Give them your IMEI number. They should be able to block your lost phone & make it unusable. The phone then becomes worthless.

No you cannot block the phone unfortunately...make sure that you call your carrier to report it lost so that you don't incurr any charges on your bill from someone using it..unfortunately it's gone or someone has put thier sim card in it and started using it...in the future it is recommended that you put a key lock and set up a pin code on your phone...and remember it because you will be the only one able to unlock it...that will save someone else from getting into your phone.
