My surround speakers are too loud. I have adjusted the speaker level, speaker distance and surround level but none of these seem to change. When i go back into menu it does reflect the changes but doesn't sound different. Any suggestions?

Did you try changing the front & center speakers to a higher level as well as setting the surrounds to a lower level? Are all speakers using the same wire gage?

How do I hook up to the surround sound

i regret to inform you - there is several thousand manufacturers of the surround sound systems... which one you wish to install and connect?

front speakers should be located about two human heights distance between them and on the height of the human...

rear speakers should be similarly located on the back of the listener.

subwoofer should be on about knee level above the floor, directly on the front, or if on the back - it should be on the floor level

twitter should be about waist level above the floor on the front, if on the back, it should be about head level.

minimal distance from the speakers to the listener should be about human height...

Only subwoofer works at high volumes

It could be your receiver but you can try seeing if the Woofer has a input knob at the back, maybe this down, or maybe.if possible turn down the amount going to the woofer with your receiver.

I have a Yamaha H/T receiver rated @ 95 watts. The front and rear speakers a hms and the center channel is 4 ohms. Is this a dangerous setup for my receiver to function in?

You seem to be confusing the power of the Receiver with the impedance. Watts is how much power or how loud it can go. While ohms is the impedance of the device. The receiver will have an lowest ohms setting for it's speakers. For example if it says 4 ohm you can connect any speaker of 4 ohms and above to it. However if it said 8 ohms then you should not connect any speaker lower than that.
Each channel will have it's own power level measured in Watts. Some units have the same level for all channels, others have different levels for the channels. You can connect any speaker that has a wattage greater than the channel's total max power. If you connect say a 50 watt speaker to a 70 watt amp, it will work, but you must never take the volume to even near the max or it will blow the speaker.
Anything with a mono soundtrack will only decode on the left and right channels. Unless the unit has something like an all channels setting. So the unit seems to be working fine. Mono films have no surround effects applied to them, so the indications are simply showing they are working. They of course feed no surround to other channels, as none are present.

I have to turn sound up to 48-49 to watch a movie and get good sound. Seems it should loud in the 20's.

Most movies are mixed at a much lower volume than CD's because there is a THX standard of 85dB which allows for greater headroom. The amount you have to turn up the gain on your amp is relative to the sensitivity of your speakers and the distance you are from them; every time you double the distance from your speaker you lose 6dB.
Most stock surround sound speakers have a lower sensitivity, meaning less SPL per watt/meter distance.

Only Direct sound works good, and only two speakers

Some sound fields have reverberant effects that need adjusting so the levels become audible, such as in "stadium" mode. While playing one of these sound fields, navigate through the speaker setup menu and make adjustments to the "effects" percentage, and levels. Be aware however, that only in 4 channel stereo mode will your surround speakers produce sound 100% of the time. Also, in any form of Dolby Pro Logic mode you will hear more sound from the surround speakers originating from stereo source material than from monoaural material...example, a television station broadcast in stereo versus mono.

Polk Audio RC55i In-Wall Surround Speakers (can't hear whispers)

I'd make sure that the receiver surround mode is set for one of the digital settings, such as Dolby Digital or DTS, and make sure that you have the center channel level adjusted to a level similar to the other two front channel speakers. I've seen/heard this happening when using dolby digital material & having surround set on something like 'hall' or 'stadium' or dolby surround, where only 4 channels are used & the amp has to 'mix' sounds to the left & right front speakers. It'll often sound like the actors on screen are talking out of the bottom of a barrel, with a hollow & tinny character to the voices.
