You need to contact your cell phone service provider, they will give you your PUK code when you provide your account details and when they have verified your identity.
PUK is sim dependent code. easy solution is to call your service providers customer care (using a different phone of course) and request for it. it is a free service.

PUKs, or personal unblocking keys, are used to unblock a mobile phone which becomes blocked.
