As you know already that format disk basically creation of FILE ALLOCATION TABLE (FAT)
NTFS (new technology file system)
it is necessary to store data on disk.
but if this error occurs while disk already formatted with any file system then simply run disk repairing utility to fix the problem
If its intel based machine or AMD and running windows then i would recommend run scandisk utility
if its mac run disk utility from utilities and check disk for any error and it will repair the disk
good luck

The rescue disc that can only be made on your laptop by your laptop contains the required drivers. The Vista DVD on has generic drivers and may have specific drivers missing. Are you attempting a fresh install or are there fragments of an old troublesome system still there? If there is fragments left, after checking that the rescue/restore data is no longer on the hard drive, format the drive first and try again. It should then work.
