As far as mounting, you may have to drill some holes in your table. Or get the quick mount from Grizzly Ind. There should be a gap between a couple of the rollers. This is where the blade sits. Dont let the blade cut into the rollers though. Set it at a very slight angle to keep the wood against the fence. Try it without cutting wood first. Hope this helps, and good luck!
If you lost the instruction manual go to dewaltservicenet.com and you can download them, but in the meantime there is a lever that removes the guard lift up on it and then towards the back is a slot for the pawls, depress the button on them and insert into the slot making sure they lock replace the guard and make sure everything moving freely up and down movement of the saw and free movement of the other safety devices. I couldn't find the exact year from your serial number only a year range which is 65-71.
Your serial # breaks down as follows.
34 = 1965 - '67: J-3000 4WD,

Truck (This means it did not come with a bed from the factory.) X = 1965 - '67: 6000# GVW or 1968 -

You should be able to narrow it down a bit more, however it's going to take comparing pictures of the various years within 65-71 and looking for changes to the style such as grills, headlights etc.
My UT21546 has 2 springs, the regular recoil one and a what I'll call a anti kickback one. the top of the anti kickback spring kept coming unsecured and finally I just deleated it alltogther and screwed the 2 plates tightly togehter and the near new unit works better than new now.
